Wonder-Time Play Lounge LLC Safety Waiver and Acknowledgment

Please fill out the following form.

1. Safety Rules and Acknowledgment:

 I, the undersigned, acknowledge that my child(s) is entering an indoor playground facility, Wonder-Time Play Lounge LLC, and agree to abide by all safety rules and regulations set forth by Wonder-Time Play Lounge LLC (in person and on our website). I understand that the facility includes various structures, equipment, and play areas, and I assume full responsibility for my child's safety while participating.

• I agree that my child(s) will be supervised at all times by a parent/guardian while in the facility.

• I confirm that my child(s) is physically capable of engaging in the activities available.

• I will ensure that my child(s) wears appropriate clothing and footwear for indoor play (e.g., no loose clothing or socks that may cause slipping).

• I understand that there are risks inherent in physical activities, including but not limited to falls, collisions, and injury, and I accept full responsibility for my child's actions and well-being.

• I agree to follow all posted rules and to ensure my child(s) refrains from engaging in any unsafe or disruptive behavior.

  
  

2. Social Media and Photo Use Consent:

 I understand that photos and videos may be taken at Wonder-Time Play Lounge LLC during my child's visit. These photos/videos may be used for promotional purposes, including on social media or marketing materials.

• I hereby consent to the use of photographs, videos, or other recordings of my child(s) for such purposes, without compensation.

• I understand that I can request that no photos or videos of my child(s) be taken by notifying the staff in advance.

  
  

3. Allergies and Medical Information:

 I hereby inform Wonder-Time Play Lounge LLC of any known allergies or medical conditions that may affect my child's participation in activities.

I acknowledge that I will notify the facility of any updates to my child's medical conditions or allergies prior to participation.

5. Liability Waiver and Release: 

By voluntarily signing this waiver, I release, indemnify, and hold harmless Wonder-Time Play Lounge LLC and its employees, agents, officers, and volunteers from any liability for injuries, damages, or claims arising from my child’s participation in activities at Wonder-Time Play Lounge LLC. I understand that while every effort will be made to ensure the safety of my child(s), Wonder-Time Play Lounge LLC is not responsible for illnesses, personal injuries, death, accidents or lost personal property.

I have read this waiver, understand its contents, and agree to the terms and conditions outlined above.
